虚拟支气管镜导航的临床应用研究进展

摘要:
周围型孤立性肺结节活检临床需求日渐增多。虽然经皮穿刺肺活检的诊断率高于经支气管肺活检,但是其气胸、出血等发生率也远高于经支气管肺活检。导航支气管镜技术的问世极大地提高了经支气管肺活检的诊断阳性率,其中的虚拟支气管镜导航因其成本低、效率高,提供了更好的选择。近年来虚拟支气管镜导航除了在周围型孤立性肺结节活检中的应用外,还与射频消融、微波消融、光动力疗法、热蒸汽消融等相结合,为未来早期肺癌患者因各种原因无法手术时提供选择替代治疗方案。本文从虚拟支气管镜导航的工作原理及方法、临床应用以及未来前景等方面进行了综述。

Abstract:
Peripheral solitary pulmonary nodular biopsy is increasingly needed.Although the diagnostic rate of percutaneous lung biopsy is higher than that of Transbronchial Lung Biopsy (TBLB), the incidence of pneumothorax and bleeding is also much higher than that of TBLB.The advent of navigational bronchoscopy has greatly improved the diagnostic rate of TBLB.Among them, Virtual Bronchoscopy Navigation(VBN) provides more choices due to its low cost and high efficiency.In recent years, in addition to the application of VBN in peripheral solitary pulmonary nodules biopsy, it has also been combined with radiofrequency ablation, microwave ablation, photodynamic therapy, thermal steam ablation, etc., to provide alternative treatment options for patients with early lung cancer who are unable to operate due to various reasons in the future.Bronchoscopic intervention is a viable alternative because it is less invasive and has more therapeutic options.This article reviews the working principle, operation method, clinical application and future prospect of virtual bronchoscopy navigation.
